It would be weird to play twinkle twinkle little star with a pick up note, but basically, a pick up note is that value taken from the last measure of the song. For example, if you have a time signature of 4/4 and the pick up note is a quarter note, the very last measure should have a total of 3 beats in it. Say your in 3/4 time and the pick up note is an 8th note... then the very last measure is worth 2 quarters and an 8th note.

Is a pick-up note considered a measure? Part of the 1st measure?
